Vendor Affirmative Action
The City of Dayton > Departments > Human Relations Council
All companies doing business with the City of Dayton that may be awarded contracts involving any expenditure or cumulative expenditures in a calendar year requiring City Commission Apprioval require an Affirmative Action Assurance (AAA) form approved by the Human Relations Council.
You may download the form and send it in by mail, fax, or e-mail. Or, you may contact the Human Relations Council for more information or to request a form. These forms expire at the end of one year and must be resubmitted annually.
