2012 HEALTH CARE BENEFITS GUIDE VIDEO

This year Health Insurance Open Enrollment takes place November 14 through November 28. Your benefit choices will be effective January 1, 2012 through December 31, 2012. It is vital that you understand which benefit choices could be best for you and your family. To assist you in making these important decisions regarding your health insurance and other insurance benefits, we urge you to watch this informational video, review the material below and attend one of the scheduled informational meetings conducted by representatives of Anthem and the Human Resources Department. Employees may drop in anytime during posted hours for assistance with benefit changes. The Benefits Open House Schedule is as follows:

  • Monday, November 14: 1:00–3:00 at Airport Terminal
  • Tuesday, November 15: 10:00–2:00 at AFSCME Union Hall, 15 Gates Street
  • Monday, November 21: 10:00–2:00 at AFSCME Union Hall, 15 Gates Street

Carefully read all of the materials in your enrollment packet. If you do not want to make any changes to your election (i.e., same plan option and same dependents covered), you do not need to do anything. If however, you want to make any changes to your plan selection or add/delete a dependent from the plan, forms will be available at the open enrollment meetings or in the Human Resources Department. All completed enrollment forms must be received by the Human Resources Department no later than November 28, 2011 at 5:00 p.m. For your convenience, you can review the your appropriate enrollment packet by clicking the links below.

If you have questions, please contact the Human Resources Department at 333-4064.
