SOCIAL SERVICE AGENCY ASSISTANCE
The Ombudsman’s office may approve an extension on the Water/Utility Billing Statement without a payment. Customers must directly contact the Ombudsman’s office at (937) 223-4613. If the Ombudsman’s office approves the extension, they will fax a letter indicating a specific date the bill shall be paid to the City’s Billing Section.
Customers can also receive assistance from Social Service Agencies. The Social Service Agency may guarantee payment in full or a portion of the balance. The Social Service Agency’s intent to assist must be in writing. The statement must include the following:
· Social Service Agency identification (statement must be on the agency’s letterhead)
· List the Social Service agency’s contact person
· Phone number of the Social Service agency
· Signature of the contact person from the Social Service Agency
· Customer’s name
· Address and/or account number for the customer
· Amount the Social Service agency will pay

A Payment Plan may be made with you with the understanding the Social Service Agency is guaranteeing the payment.
The Social Service agency has up to 30 days to submit payment.
Failure to pay by the dates agreed upon, by the Social Service Agency or customer, will result in water being shut-off without notice.
